The following documents must be provided for all NEW students enrolling at Northbridge High:

• District Student Registration Form or Online Registration ID #

• Birth Certificate

• Proof of Residency: Mortgage payment record, copy of lease and most recent payment, completed Landlord Affidavit, or Section 8 agreement,AND gas, oil, electric bill, water/sewer bill, home (landline) telephone bill, or excise tax bill, AND, valid MA driver's license, valid MA photo ID, valid

  passport or green card, AND signed Residency Statement or Residency Affidavit. *

• Most recent Physical and Immunization Record

• Legal Proof of Guardianship or Custodial Paperwork-this documentation must be provided if you fall into one of the categories below:

  • If you are not listed as the mother/father on the student's birth certificate *

  • If you are not the mother/father, but have physical custody of the student *

  • If either the mother or father have joint legal custody with physical custody going to either mother or father *

  • If either the mother or father have sole custody *

(Packets are also available in the Guidance Office at NHS).

Return the completed packet to Lori Susienka in the Guidance Office. During the school day, you may register between 7 am and 2pm.  During the summer, it is best to call and make an appointment.

**If you choose to register online, you will need to print, complete, and return an OLR Supplemental Packet. This will be submitted to the Guidance Office with all required documents listed above.
