Print out this form. Have you new employer fill out and sign the first page. Parents sign the 2nd page and bring papers to the High School office to complete. If you are under 16 years of age, your doctor must also sign. If you do not go to our school please bring in your attendance record from your school for at least the last 100 days and a copy of your birth certificate.
